దీనిని నింపడానికి ముందు certificate రాని ఉపాధ్యాయులు, దీక్ష app లోకి వెళ్లి profile ను screen shot తీసుకుని, ఈ form ను నింపి, screenshot ను upload చేయవలసి ఉంటుంది.
Modules 1,2,3 issues పూర్తి అయిన తర్వాత, next Modules కు ఈ form update చేయ బడుతుంది.
